What We Offer

Desert Care Hospice provides a personalized program of physical, emotional, and spiritual end-of-life care. We aim to control the patient’s pain and other symptoms, and at the same time understand their concerns and wishes, so that they can still live their lives to the fullest.

At Desert Care Hospice, we put both the patient and the family at the center of what we do. We also work closely with the patient’s primary caregivers and physicians to ensure that the patient’s needs are met. We also coordinate with insurers, community organizations, and facilities to address the different burdens that comes with life-limiting and terminal illnesses.

Desert Care Hospice provides the four levels of hospice care. A patient can change to different levels based on a patient’s changing needs. The needs of the patient, and the physician’s evaluation of the medical condition, will determine the level of care needed. These are the four levels of hospice care:

ROUTINE HOSPICE CARE – Routine Hospice Care is the most common level of hospice care. This is for patients whose symptoms are well-managed and under control.

This level of care uses the basic visit schedule from our interdisciplinary team. The frequency of visits depends on the goals of the individual plan of care.

Desert Care Hospice can provide routine hospice care services at the patient’s residence or wherever the patient considers as his/her home.

This also includes 24/7 access to Desert Care Hospice’s triage team to answer any questions or address symptoms that may arise.

CONTINUOUS CARE – When a patient’s symptoms become severe and need ongoing interventions, Continuous Care can provide a level of care to help manage the patient’s pain and other acute symptoms at home.

Desert Care Hospice can provide hospice nurses and nursing aides at bedside for up to 24 hours a day, in close consultation with the hospice physician.

This level of care may be provided at the patient’s residence, assisted living facility, or care home.

GENERAL INPATIENT CARE – When a patient’s symptoms cannot be managed at home and need careful monitoring and intervention, General Inpatient Care may be considered.

This level of care takes place in a skilled nursing facility or hospital. Desert Care Hospice has arrangements with hospitals and other licensed facilities so that we can provide patients the necessary care round the clock.

RESPITE CARE – At Desert Care Hospice, we understand that caregivers need relief too. Respite Care is designed to temporarily take over caregiving duties for up to 5 days.

Take that much needed break with a peace of mind as we provide care for your loved one at home or in a licensed facility.

Our Hospice Team

Hospice care is a team effort. Our team works closely with the patients and their families to develop and execute a personalized plan of care that meets the needs of each patient.

Each member of our interdisciplinary team is specially trained for hospice care. Here is our team at your service:

Physicians – Our physicians specialize in pain and symptom management who collaborate with our hospice team to oversee the plan of care.

Nurses – Our nurses provide hands-on care and are experienced in pain management.

Hospice Aides – Our certified hospice aides are specially trained and available to assist patients with personal care and daily living activities.

Dieticians – Our dietician can diagnose and treat nutritional problems. They can alter a patient’s nutrition based on their medical condition and individual needs.

Social Workers – We have skilled social workers who provide emotional support for the patients and their families. They can also assist with the utilization of community resources, insurance and facility issues, financial matters, and funeral arrangements.

Chaplains/Bereavement Counselors – Our spiritual care counselors provide non-denominational faith and bereavement support to patients and their families.

Volunteers – We have volunteers who are specially trained to provide friendly visits for companionship and relief care.

Other Services

Depending on the needs of each patient, Desert Care Hospice may provide other services related to the patient’s hospice diagnosis, such as medications, supplies, and medical equipment.

Aside from educating the family or caregiver on how to care for the patient and giving emotional, psychosocial and spiritual support, Desert Care Hospice also provides faith and bereavement support to patients and their families.

Our hospice care services are available 24 hours a day, 7 days a week, with on-call staff for urgent needs.
